Most patients are evaluated through one lens at a time.
An orthopedic evaluation may focus on joints, discs, tendons, ligaments, imaging, and
structural diagnoses.
A functional medicine evaluation may focus on inflammation, hormones, nutrition,
immune function, metabolism, and whole-body physiology.
But chronic pain rarely comes from one system alone.
Orthopedic Functional Medicine brings these worlds together.
The Orthopedic Functional Medicine Evaluation is the diagnostic foundation of The Altman Method. It combines advanced orthopedic analysis with functional medicine principles to determine why the body is not healing and what system needs to be treated first.
The Four-Lens Evaluation
This evaluation looks at how four major systems interact:
Structure
Joints, ligaments, tendons, fascia, discs, posture, alignment, biomechanics,
movement mechanics, and tissue integrity.
Nervous System
Proprioception, balance, coordination, protective guarding, pain sensitivity, vagus
nerve function, autonomic regulation, and post-concussion patterns.
Chemistry
Inflammation, hormones, immune function, nutrition, metabolic health, medication
history, prior procedures, and lab findings that may affect healing.
Energy
Circulation, oxygen delivery, mitochondrial function, cellular voltage, sleep quality, recovery capacity, and tissue readiness for repair.
The goal is not simply to name a diagnosis.
The goal is to understand why the body is stuck — and what needs to change for healing to
become possible.
Why the Right Evaluation Matters
A diagnosis should explain more than where pain is located.
It should explain why the problem is happening.
Many patients are told they have arthritis, tendonitis, a disc injury, muscle tightness, neuropathy, or chronic pain — but that label does not always explain why symptoms started, why treatment failed, or why the pain keeps returning.
When the true driver is missed, care often becomes a cycle:
Temporary relief.
Symptoms return.
Another medication.
Another injection.
Another round of therapy.
Another scan.
Another explanation that still does not fully make sense.
